These documents will be required before the start of the new school year: 

Proof of Age: Original birth certificate, passport, hospital birth record, or duly attested baptismal record Immunization Record: 5-DTap, 4-Polio, 3-Hepatitis B, 2-MMR, 2-Varicela Physical Examination (Kindergarten only) Dental Exam (Kindergarten only)

Legal Custody or Court Orders (if applicable) Proof of Address (two required): Current rental agreement Lease agreement Utility bill (electricity, landline telephone or cable), etc. Must include parent's name Cell phone bill or driver's license may not be used for valid proof of address
