These documents will be required before the start of the new school year:

Proof of Age:
- Original birth certificate, passport, hospital birth record, or duly attested baptismal record
Immunization Record:
- 5-DTap, 4-Polio, 3-Hepatitis B, 2-MMR, 2-Varicela
Physical Examination (Kindergarten only)
Dental Exam (Kindergarten only)

Legal Custody or Court Orders (if applicable)
Proof of Address (two required):
- Current rental agreement
- Lease agreement
- Utility bill (electricity, landline telephone or cable), etc. Must include parent's name
- Cell phone bill or driver's license may not be used for valid proof of address
